THE APPROACH
What is Continuous / Extended Care?
Continuous / Extended Care refers to oral antifungal treatment that continues beyond the initial 90-day framework — sometimes for several additional months or longer — under Florida-licensed provider supervision. Because toenails grow slowly and some fungal infections respond gradually, certain cases may benefit from continued or extended care. Extended care is never automatic; it is determined by the provider on a case-by-case basis, based on clinical response, condition severity, patient history, contraindications, and ongoing monitoring findings including liver function testing (LFT).
THE PROTOCOL
How Continuous / Extended Care may work
Phase 1
Initial Course
The standard 90-day framework is completed under provider supervision.
Phase 2
Clinical Review
The provider evaluates clinical response, nail growth, tolerance, and monitoring results.
Phase 3
Continuation Decision
If clinically appropriate, the provider determines whether extended care is warranted; otherwise, treatment is completed or an alternative approach is considered.
Phase 4
Extended Treatment
Continued oral antifungal treatment under ongoing supervision, with medication, dosing schedule, and duration determined by the provider based on the individual case.
Phase 5
Ongoing Monitoring
Repeat liver function testing (LFT) and clinical follow-up as directed; treatment is continued, modified, or ended based on provider judgment.

Case-by-Case Continuation
Extended care is never automatic; each continuation is a separate provider decision based on clinical response and medical review.
Ongoing Safety Monitoring
Longer treatment durations require ongoing safety monitoring, including repeat liver function testing (LFT), to help identify concerns early.
Provider-Directed Supervision
Every phase of extended care is directed by a Florida-licensed provider within JAWS Podiatry; patients cannot self-continue treatment.
IS IT RIGHT FOR YOU?
Who may be considered for Continuous / Extended Care
✔ MAY BE APPROPRIATE FOR:
✔ Eligible adult patients who have completed an initial 90-day course and require additional treatment based on provider assessment
✔ Patients with severe, multi-nail, or slow-responding fungal toenail cases as determined by the provider
✔ Patients for whom the provider judges that continued treatment is clinically appropriate
✔ Patients who can complete extended follow-up and safety monitoring as directed
✘ MAY NOT BE APPROPRIATE FOR:
➝ Patients with contraindications to continued oral antifungal medication
➝ Patients whose safety monitoring (including LFT) shows concerns that make continuation inappropriate
➝ Patients whose case may be better managed with an alternative approach (topical, in-person evaluation, or specialist referral)
➝ Patients who cannot complete required follow-up monitoring
➝ Patients under 18 years of age
