STANDARD ELIGIBILITY PATH
Start your treatment eligibility review at no additional service fee.
ADDITIONAL SERVICE FEE
The Standard Eligibility Path is the standard way to begin the Fugly Toenails™ treatment process without paying for an additional consultation or case-review service. Start by completing our secure eligibility screener with the requested information about your Florida residency and physical location in Florida, age eligibility, symptoms, medical history, medications, and other relevant health factors.
If the screener determines that you meet the preliminary eligibility criteria, you may continue through the standard intake process and provide the requested health information, photos, and consents. Your case will then proceed to review by a licensed podiatric provider.
The provider reviews the information you submitted to determine whether a Fugly Treatment Plan is clinically appropriate for your case. When appropriate, the provider may issue the prescription required for the selected treatment plan. The final decision to prescribe depends exclusively on the provider’s clinical evaluation and is never guaranteed by the eligibility screener.
If the screener determines that you do not meet the preliminary eligibility criteria, your case will not proceed to provider review within the Standard Eligibility Path, and no prescription will be issued through this pathway.
Important Service Clarification: $0 means no additional service fee — not free treatment.
There is no separate consultation or case-review fee to begin through the Standard Eligibility Path. If a Fugly Treatment Plan is prescribed, the treatment plan is purchased separately at the applicable price. The Standard Eligibility Path does not include a dedicated written clinical report or a live video consultation. Provider Case Review ($99) and Direct Video Consultation ($249) are separate optional services for patients who want a more in-depth written clinical review or direct live interaction with a provider. Fungal Nail Diagnostic Testing is separate from the Standard Eligibility Path and is not required for every treatment plan. When additional laboratory confirmation is clinically appropriate, a provider may recommend testing. The current laboratory fee is US$125, with payment and kit delivery handled directly by the laboratory.

ABOUT THIS PATHWAY
What is the Standard Eligibility Path?
The Standard Eligibility Path is the primary way to begin the Fugly Toenails™ telemedicine process without paying an additional service fee. It begins with an eligibility screener designed to collect information relevant to your condition, medical history, and preliminary eligibility.
If you meet the preliminary eligibility criteria, your case proceeds to review by a licensed provider, who determines whether a Fugly Treatment Plan — Fugly Complete™, Fugly Topical™, or Fugly Nail + Skin™ — is clinically appropriate. When appropriate, the provider may issue the prescription required for the selected treatment plan.
This provider review is part of the standard eligibility and treatment process and does not include a separate in-depth case review, written clinical report, or live video consultation. There is no separate consultation or case-review fee. The screener provides preliminary eligibility only; final treatment and prescription decisions remain with the licensed provider.
What's included in the Standard Eligibility Path.
✓ Access to the Fugly Toenails™ Eligibility Screener
✓ Preliminary eligibility screening based on the information you provide
✓ Provider review when preliminary eligibility criteria are met
✓ Clinical review of your individual case by a licensed podiatric provider
✓ Determination of whether a Fugly Treatment Plan is clinically appropriate for your case

How It Works (4 Simple Steps).
Step 1
Complete the Eligibility Screener
Submit the requested information about your symptoms, medical history, medications, Florida residency, and other eligibility factors.
Step 2
Complete Your Intake
If you meet the preliminary eligibility criteria, provide the requested health information, photos, and consents so your case can proceed to clinical review.
Step 3
Provider Review & Treatment Determination
A licensed provider reviews your submitted case information to determine whether a Fugly Treatment Plan is clinically appropriate. When appropriate, the provider may issue the prescription required for treatment.
Step 4
Treatment Plan & Fulfillment
If treatment is prescribed, you can move forward with the Fugly Treatment Plan selected for your case. Treatment plan pricing is separate, and fulfillment is handled through an authorized dispensing pharmacy, as applicable.
Note: If the screener determines that you do not meet the preliminary eligibility criteria, your case will not proceed to provider review within the Standard Eligibility Path.
PROVIDER-DIRECTED OUTCOMES
Your next step depends on your individual case.
✔ Fugly Treatment Plan
If the provider confirms that treatment is clinically appropriate, a Fugly Treatment Plan may be prescribed based on your individual case.
✔ Diagnostic Testing
Fungal Nail Diagnostic Testing may be recommended in selected cases when additional laboratory confirmation is clinically appropriate.
✔ In-Person Care
If your condition cannot be appropriately evaluated or treated through telemedicine, in-person medical care may be recommended.

CLINICAL OVERSIGHT
Provider-Directed Clinical Care.
Fugly Toenails™ is operated by JAWS Podiatry in Hollywood, Florida. The eligibility screener provides a preliminary determination only. Final treatment and prescription decisions are made by the licensed provider based on the individual case and clinical judgment.
