Clinical Review and Prescription Decision
If the screener determines that you meet the preliminary eligibility criteria, a licensed provider will review the information you submitted to confirm whether treatment is clinically appropriate. When appropriate, the provider may issue the prescription required for the selected Fugly Treatment Plan.
If the screener determines that you are not eligible, your case will not proceed to provider review within the Standard Eligibility Path, and no prescription will be issued.
The Standard Eligibility Path does not include a dedicated written clinical report or a live video consultation. The final decision to prescribe depends exclusively on the provider’s clinical evaluation.

No. You can begin with the Standard Eligibility Path at $0 Additional Service Fee. The Provider Case Review ($99) and Direct Video Consultation ($249) are optional services for patients who want a more in-depth written review or a live discussion with a provider.
Q: Are consultation fees included in the Fugly Treatment Plan price?
No. The $99 Provider Case Review and $249 Direct Video Consultation are optional services and are charged separately from your Fugly Treatment Plan.
The Standard Eligibility Path does not require an additional consultation fee.
Q: Can I choose which Fugly Treatment Plan I want?
You may review the available treatment plans and pricing, but the appropriate treatment pathway is determined by a licensed provider based on your medical history, symptoms, photos, eligibility, and clinical appropriateness.

The Provider Case Review ($99) provides a dedicated, more in-depth review of your submitted information with a written clinical recommendation. No live video appointment is required.
The Direct Video Consultation ($249) is a scheduled 30-minute live video consultation with a licensed provider, allowing you to discuss your case and ask questions directly.
Q: Is Fungal Nail Diagnostic Testing included in my treatment plan?
No. Fungal Nail Diagnostic Testing is a separate laboratory service and is not required for every treatment plan.
When additional diagnostic confirmation is clinically appropriate, your provider may recommend testing. The current laboratory fee is US$125, and both payment and kit delivery are handled directly by the laboratory.
Q: Do I have to complete diagnostic testing before starting treatment?
Not necessarily. Diagnostic testing is recommended only in selected cases when your provider determines that additional laboratory confirmation is clinically appropriate.
